Job summary
The Nursing Associate will be an integral member of the Community Mental Health Team, supporting the Community Non-Medical Prescriber (NMP) Nurse and multidisciplinary team to deliver safe, effective, high-quality care. They will contribute to medication oversight, physical health monitoring, health promotion, and person-centred care for service users with complex mental and physical health needs.
Working under the supervision of registered practitioners, the post holder will undertake physical health assessments, monitor and report clinical observations, support medicines management, and contribute to care plans. They will work with service users, carers, primary care, and partner agencies to promote holistic care, identify physical health concerns early, and address medication-related issues.
