Job Summary Provide comprehensive primary care to patients of all ages, delivering evidence-based diagnosis, treatment, and preventive services. Coordinate patient care with specialists and allied health professionals while maintaining accurate clinical documentation. Number of Positions 3 Main Duties Obtain detailed medical histories, perform physical examinations, and order or interpret diagnostic tests to establish diagnoses. Develop, implement, and adjust individualized treatment and care plans, including prescribing medications and therapies. Deliver preventive care services such as routine screening, immunizations, health promotion, and lifestyle counselling. Manage acute illnesses and exacerbations of chronic conditions, and perform minor clinical procedures as appropriate.
