Night-Shift USRN Care Coordinator — Chronic Care Management

VISAYA in Bacolod is seeking a dedicated Care Coordinator with a strong nursing background to join their healthcare team. The ideal candidate should have clinical experience, particularly in case management, and must be willing to work onsite on a night shift schedule.

This role involves managing chronic care plans, monitoring patient health, collaborating with healthcare providers, and providing education to patients and their families. Candidates with previous BPO experience are preferred but not required.
