Minneskoordinator: Samordna vård för minnespatienter

Varsinais-Suomen hyvinvointialue is seeking a memory coordinator to support patients with memory disorders and their relatives throughout the care pathway. The role involves monitoring care plans and coordinating between patients, families, and professionals to ensure timely and accurate treatment and rehabilitation.

You will conduct memory assessments, participate in planning and follow-up, and work within a broad multidisciplinary network, including doctors, memory nurses, home care, and
