Medical Social Worker - Holistic Care & Crisis Support

PVH (Tommy Hilfiger/Calvin Klein) invites applications for a Medical Social Worker based in Bright Hill, Singapore. The role supports intake, psychosocial assessments, discharge planning, ACP, bereavement and resident engagement, working Monday to Friday from 8.30am to 6pm.

The ideal candidate holds a Bachelor Degree in Social Work, is registered with SWAAB, and has 3–5 years’ experience in hospital or community hospital settings with elderly/dementia exposure.
