Medical Social Worker: Elder Care & Palliative Support

The role covers intake, psychosocial assessment, case management and Advance Care Planning to support residents and families in a hospital or community hospital setting. You will conduct assessments, provide counselling, coordinate with healthcare and government agencies, and participate in discharge planning and bereavement support.
