Macmillan Cancer Social Prescriber

In partnership with Macmillan Cancer Support, we are recruiting a Macmillan Cancer Social Prescriber (Band 4) to support delivery of the Improving my Cancer Journey (ICJ) service within the Primary, Community and Intermediate Care (PCIC) Directorate across Cardiff and the Vale. This role supports people affected by cancer through a non-clinical, community-based social prescribing approach, working primarily within primary care and community settings. The postholder will act as a friendly point of contact from referral, holding personalised conversations to understand what matters most to people and supporting them to access community, voluntary sector and wellbeing services alongside their cancer care. Approved ICJ conversation tools will be used in line with agreed protocols to support consistent, high-quality non-clinical discussions.
