***NOTE: This is a hybrid-remote role with 75% local home visits focusing on Physically Disabled/Elderly waiver services in southern Illinois. Preference will be given to applicants with past case management, advocacy or home visits/community travel experiences **who are located in the following Southern Illinois Counties: Johnson or Union (nearby cities include Vienna or Anna-Jonesboro).**
