The LPN - Supportive Care Liaison II identifies palliative care needs and facilitates transitions to hospice when appropriate. This role partners with clinical teams and palliative operations to conduct telephonic assessments, educate patients and families, complete referrals, support care coordination and promote strong interdisciplinary collaboration.
