Job Summary

Responsible and accountable for the ongoing teaching, data collection regarding the condition and care, safety and monitoring of chronic hemodialysis patients as well as providing support and education to the patient’s family. Functions as an intricate member of the multi disciplinary team.

Responsibilities

• Maintains a therapeutic environment through ongoing teaching, data collection and intervention in appropriate response to the patient’s age, size and medical condition.


• Performs and documents ongoing monitoring of the Dialysis patient. Participates in patient care plans, development of educational needs and evaluation of patient outcome.


• Reports changes in patient care needs to the hemodialysis charge nurse or designee.
