**Licensed Practical Nurse (LPN) Case Manager**
**Location:** **Northeast MA (Reading, Lowell, Lawrence, Salem area)**
**Schedule:** **Monday–Friday, 9:00 AM–5:00 PM**
**Type:** **Full-Time | 13-Week Contract (Temp-to-Perm or Direct Hire)**
**Pay:** **$47/hour + $0.72/mile reimbursement**

**Amergis Healthcare Staffing is partnering with an Adult Day Health organization seeking an** **LPN Case Manager** **to support elderly adults and individuals with disabilities in the community. This role focuses on client assessments, care plan management, caregiver support, and care coordination through regular home visits.**
