Hudson Pointe at Riverdale Center for Rehabilitation & Nursing is currently seeking a Full-Time RN LPN's for day (7am-3pm) shift and evening (3pm-11pm) to join our dynamic organization.

OVERVIEW:

Under the leadership of the Registered Nurse, the LPN will be providing coordinated care to residents in a skilled nursing facility.

RESPONSIBILITIES:

Provide direct care responsibilities to patients

Supervise CNA s

Administer medications and treatments timely and according to policy

Receive, transcribe, and carry out physician orders

Follow established procedure for charting

Complete patient assessments, coordination of care

Identify and reports changes in condition

Follows environmental health & safety and sanitation standards and procedures

Oversees clinical and non-clinical acute operations and continuum of care
