Job Description
Insight Global is looking for a Lead Case Manager for one of their top social services clients in South Jersey.
This role focuses on providing direct counseling and case management services to high-risk, unaccompanied youth while supporting their journey toward safe reunification with appropriate sponsors. On a day-to-day basis, the individual manages an assigned caseload overseeing 4 caseload, develops and updates personalized reunification plans, and ensures each youth receives comprehensive, integrated services, including legal, medical, and trauma-informed care. They regularly meet with youth, at minimum weekly, and coordinate with internal teams, external agencies, and sponsors to monitor progress and move cases toward timely release, typically within 30 days.
