Sentara Norfolk General Hospital is hiring an Inpatient Case Manager RN to work part-time 20 hours per week.
This is a weekend day shift position!
The Inpatient Case Manager RN is responsible and accountable for the provision and facilitation of comprehensive care management, services and quality outcomes for patients across the healthcare continuum, primary focus on patient transition to the appropriate level of care while facilitating clinical appropriateness and LOS. Directly support those in hospital patients identified in need of comprehensive case management.
