In-Home LPN Care Coordinator

Ennoble Care is seeking an LPN to provide in-home primary, palliative, and hospice care coordination. The role involves developing personalized care plans, coordinating with patients and primary care clinicians, and documenting all communications using EMR systems.

The ideal candidate will be comfortable on the phone for extended periods, capable of multitasking across patients and workstreams, and able to pass background checks and drug screening. Weekend and weekday schedules may apply.
