What You Need to Know
AccentCare is seeking a hospice field visit physician that can provide compassionate, patient-centered medical care to individuals with serious, advanced, or terminal illnesses. This role is responsible for conducting hospice and/or palliative care visits, evaluating patients for hospice eligibility and recertification, managing pain and symptoms, and supporting patients and families through goals-of-care and advance care planning discussions.
Job Responsibilities:
+ Evaluate patients for pain, symptom management, and hospice eligibility (admissions and recertifications).
+ Counsel patients and families on care options and advance care planning.
+ Order tests, initiate treatment, and conduct face-to-face visits as needed.
