HCS Program Coordinator/Case Manager

We are seeking a full time HCS Program Coordinator/Case Manager for our HCS Department

BASIC FUNCTION:

Conducts analysis and assessment of HCS programs, processes and outcomes utilizing quantitative tools and methods to identify performance issues and interventions to be utilized throughout HCS. Develops, analyzes and manages Health Care Services (HCS) department outcome projects to ensure contractual compliance is maintained. Produces outcome and program reports and makes recommendations for improvement. Develops and maintains department Quality Improvement Projects.

Key Responsibility Areas:
