Geriatric Medical Social Worker & Care Navigator

THE SUPREME HR ADVISORY PTE. LTD. is seeking a Medical Social Worker in Singapore to support intake, assessments and discharge planning for residents and families. The role includes counselling, case management, ACP facilitation and coordination with nursing and community resources.

The ideal candidate holds a Bachelor in Social Work, SWAAB registration, and 3–5 years of hospital experience, with dementia exposure. Certification in ACP is a plus and a supportive multidisciplinary team awaits.
