Geriatric Care Social Worker | Case Management & ACP

The Supreme HR Advisory Pte Ltd in Bright Hill, Singapore, is seeking a Social Worker to join our hospital/community hospital team. The role involves intake screening, psychosocial assessments, and coordinating referrals with nursing staff.

You will provide counselling to residents and families, facilitate Advance Care Planning, and support discharge planning and community resource links. Office hours are Monday to Friday, with a focus on resident-centred care.
