Geriatric Assessment & Care Coordinator

Singapore General Hospital is seeking a compassionate and organised individual to support comprehensive geriatric assessment services, working with geriatricians, nurses, allied health professionals, patients, caregivers, and community partners to ensure holistic and coordinated care for older adults.

You will conduct outpatient geriatric cognitive assessments, gather and document patient information, and provide patients and caregivers with information on cognitive health and available support
