Responsibilities

• Perform comprehensive in-home medical assessments and consultations for patients
• Diagnose and manage a variety of acute and chronic medical conditions
• Develop individualized care plans tailored to meet each patient’s unique health requirements
• Educate patients and their families on health management, treatment options and preventive care
• Prescribe medications and perform necessary procedures as part of treatment plans
