Family Support Case Specialist - 2025784

POSITION SUMMARY:

This position will facilitate group classes for caregivers, assist program participants in gaining access to community resources, and work with volunteers to help clients adjust to life in the U.S. Additionally, the position will focus on supporting case managers as well as provide Medicaid funded case management service. A successful candidate will demonstrate a proven ability to assist immigrants in achieving stability and overcoming barriers that hinder healthy integration. This position is grant funded.

ROLE & RESPONSIBILITIES:

Group Services

Facilitate topic-specific group sessions for a specific number of individuals in caregiving roles annually, covering topics such as self-care, health promotion topics, psychosocial education, and caregiving resources.
