Discharge Planning Associate, Addiction Medicine

UPMC McKeesport is searching for a full-time Discharge Planning Associate to assist with covering the Addiction Medicine Unit!

Are you a social worker or nurse interested in care management, case management, or care coordination? UPMC is proud to announce the Clinical Care Coordination and Discharge Planning team, dedicated to caring for patients throughout their UPMC treatment journey. This full-time role will work 4, 10-hour weekday shifts from 7am-5pm or 8am-6pm but would occasionally rotate through weekend and holiday coverage as needed. You'll assist in covering our UPMC McKeesport 3C Addiction Medicine Unit, which helps patients on their journey to recovery from drug and alcohol addiction.
