Diabetes Educator RN

**Regular, Full-Time position at the Diabetes Wellness Center in Talihina | Schedule: Mon-Fri 8am-4:30pm. This is a float position that will rotate between RN and Diabetic Educator duties, as needed | Pay: Paygrade 13, Hourly, Dependent upon experience**

**Job Purpose or Objective(s):** The Diabetes Educator provides nursing assessment, care, and education to diabetic patients and their families. You will report to the Manager or Director of your department.
**Primary Tasks:**
1. Provide diabetes self-management education including healthy coping, healthy eating, problem solving, being active, taking medication, monitoring, and reducing risks.
2. Complete comprehensive assessments for each patient, interpret personal health data, and develop an individualized plan of care based on needs and goals.
