GENERAL SUMMARY
Provides direct care and coordination of medical services to program participants.
ESSENTIAL JOB FUNCTIONS*
+ Provides direct care, as needed.
+ Provides training in relevant health topics for staff and clients, as needed.
+ Coordinates medical services with each participant’s primary physician or medical clinic.
+ Oversees all health care services provided to program participants.
+ Participates in the ongoing interdisciplinary evaluation and review of each participant’s needs and status.
+ Obtains reports and approval of treatment plans from primary physicians.
+ Documents recommendation in participants’ records resulting from periodic reviews.
+ Advises program management and staff of any medical problems that may hinder a participant’s progress or that may be negatively impacted by program activities.
