The Care of the Elderly Department is currently expanding in order to meet the needs of an increasingly frail and complex population. The Trust works with the Complex Care team and Primary Care Networks (PCNs) who seek to address the needs of their most complex patients and aims to prevent admissions by meeting needs in the community before the patient reaches crisis point.
