In this replacement role, you will be responsible for assessing patients of various ages and complex care needs. Delivering high-quality care in their residences, you’ll build therapeutic relationships and ensure comprehensive patient support. Morning shifts from 8 am to 4 pm include every third weekend off.

• Assess patients’ individualized needs with appropriate tools

Requirements:
• CNO registration as a Registered Nurse in good standing
