Community Nurse: Care Coordination & Staff Training (Travel)

NTUC Health Co-operative Limited is seeking a Community Nurse to plan, coordinate and deliver in-home and community care for the elderly. You will conduct assessments, implement care plans, and provide on-the-job training alongside the Nurse Manager.

Travel across Singapore to visit patients, educate caregivers, uphold infection control standards, and contribute to ongoing professional development through supervision and learning activities.
