Job summary
The Community Learning Disability Service provides integrated specialist health and social care for adults with learning disabilities in Tower Hamlets. The service also manages a specialist day activity project called CREATE.

Key Responsibilities

• Utilize specialist nursing skills to work with individuals with learning disabilities, focusing on the maintenance, promotion, and improvement of their health.
• Support people with learning disabilities in reaching their full potential by minimizing the impact of their impairments, which could hinder their social inclusion. This will be achieved through a person-centered approach that promotes choice, independence, and inclusion.
• Facilitate access to healthcare services by collaborating and partnering with primary and secondary health services, as well as other relevant organizations.
