Job summary
We are looking for a highly motivated and compassionate Community Learning Disability Nurse to join our multidisciplinary Learning Disability Team.

This is an exciting opportunity to deliver specialist, health interventions to adults with learning disabilities, supporting them to achieve better health outcomes and improved quality of life. You will work across a variety of settings, including people's homes, day services and health settings, providing holistic specialist health assessments and interventions to adults with complex health needs.

• Undertake comprehensive specialist assessments and develop personalised care plans
