Job overview

The Mental Health Crisis Nurse Practitioner (Band 6) is an established clinical post within the City and Hackney Home Treatment Team, operating as part of the wider Crisis Pathway framework. The post holder will deliver urgent community crisis assessments, working within a multidisciplinary team to provide person-centred care in the least restrictive environment.

The post holder will carry out face-to-face mental health and risk assessments, agree crisis safety care plans with service users, carers, and families, and deliver psychosocial interventions within the Recovery Model to support self-management and recovery.

The role requires liaison with Primary Care, Community Mental Health Teams, Social Services, In-Patient services, the Voluntary Sector, and Criminal Justice.
