Responsibilities

• Patient Assessment – Evaluate patients’ mental, emotional, and social needs, including family dynamics, financial situation, and support systems.
• Care Planning – Develop and implement care or discharge plans in coordination with doctors, nurses, and other healthcare staff.
• Counseling & Emotional Support – Provide guidance to patients and families dealing with stress, trauma, chronic illness, or end‑of‑life situations.
• Discharge Coordination – Arrange post‑hospital care such as rehabilitation, home healthcare, or placement in long‑term care facilities.
• Resource Connection – Link patients with community resources, including financial aid programs, housing services, or support groups.
• Advocacy – Ensure patients’ rights are respected and help them navigate healthcare systems and insurance processes.
