Clinical Service Coordinator – Older Adults & Disability Care

Health New Zealand | Te Whatu Ora in Hamilton invites applications for a permanent full-time Health of Older People role at Disability Support Link. You will assess needs, coordinate services, and work with clients, families and communities to support optimal health and well‑being.

The position involves partnership with older people with disability needs and those with long‑term health conditions, across community and inpatient settings, with training provided where required.
