Clinical Pharmacist in GP Practice

We are a well established GP training practice serving approximately 8000 patients with 7 GPs and an ANP. We are seeking an enthusiastic and motivated Clinical Pharmacist Independent Prescriber to join our friendly and supportive team on a part-time basis of 16-20 hours per week with flexible working hours. There is already a pharmacy team supporting the practice employed by the health board who manage hospital discharge letters, secondary care prescriptions and acute script requests. The successful candidate will play a key role in supporting the management of long-term conditions and medication optimisation including;

-Hypertension reviews and medication titration

-Lipid management and statin prescribing

-Chronic Kidney Disease management

-Diabetic medication reviews and optimisation

-Asthma and COPD reviews
