The Community Registered Dietitian will act as a coordinator to serve the needs of ambulatory clients who are identified as being food insecure by the Hunger Vital Signs™ or designated screening tool and have been referred for services. The dietitian will serve as the coordinator to connect clients with chronic disease who may have limited access to food in their own communities to the hospital's designated Food Pantry.
