Foundations San Francisco Intensive Outpatient Program (IOP) is looking for a dynamic and talented Clinical Care Coordinator. Located in the heart of the Embarcadero district, Foundations San Francisco offers comprehensive, integrated outpatient treatment in a convenient urban setting. Flexible daytime and evening programs address addiction and mental health issues in a supportive atmosphere.

The Clinical Care Coordinator tracks clients from the intake call through the admissions process and episode of care including general oversight of case management, aftercare planning, and discharge. The position provides primary interfacing with referents, family members, and the clinical team. Incumbent works closely with the Director to ensure smooth day-to-day operations and may fill in as group facilitator or assessor when necessary.
