CHC Service Coordinator Supervisor- Southwest, PA (Hybrid)

Community HealthChoices (CHC) is Pennsylvania's managed care long-term services and supports (LTSS) program serving seniors and individuals with physical disabilities in the Commonwealth who are covered by Medicare and Medicaid.

Provides service coordination services, according to the level of need. The Service Coordinator Supervisor will be responsible for the performance of the General Service Coordination duties as well as the specific duties as assigned to each level of service.

We are seeking a Service Coordinator Supervisor to support the Southwest region in Pennsylvania. This is a full time position working Monday through daylight areas and will be considered a hybrid position.
