In this role, you will support individuals with disabilities by coordinating all services for your assigned caseload. You will partner with Community Connections in their mission to provide optimal independence and enriching services to people with disabilities. We are looking for someone with a commitment to compassion, promoting accessibility and serving the community.

What You'll Do:

  • Be responsible for Consumer Case Management
  • Coordinate Individual Service Plans 
  • Work in conjunction with Developmental Specialists in developing and documenting all ISP goals and objectives
  • Develop and maintain all case management records for assigned caseload
