Case Manager (RESTORE Intensive Family Services)

The Role

As a Case Manager within Centacare’s RESTORE Intensive Family Service, you will play a key role in strengthening children’s safety, wellbeing, and connection by working alongside families experiencing multiple and complex challenges. Guided by traumainformed, strengthsbased and familycentred practice, you will provide therapeutic case management and practical inhome support to help families create safe, stable and nurturing environments.

Working as part of a supportive multidisciplinary team, you will build meaningful relationships with parents, carers and children, completing assessments, developing collaborative case plans, and delivering therapeutic and practical interventions tailored to each family’s needs.
