Description
Position Title: Ithaca Shelter Case Manager
Reports to: Program Manager-Ithaca
Position Summary The Case Manager works directly with residents of the single adult emergency shelter to support them in developing individualized goals and successfully transitioning from homelessness to permanent housing. This role emphasizes person-centered, strengths-based, and trauma-informed care practices, ensuring that all services are delivered with dignity, respect, and in accordance with agency guidelines and professional standards in the social work field.
