Case Manager
The Opportunity
The Case Manager will deliver youth-focused gun violence prevention and community violence intervention programming for FCA's Empowerment Through Wellness to Prevent Gun Violence Initiative.
Responsibilities
+ Work as an onsite case manager and member of the team to support youth charged with gun-related offenses.
+ Provide individualized case management for all participants.
+ Make appropriate cross-system referrals i
+ Life Skills: Harm reduction, parenting, healthy habits, financial literacy, and planning.
+ Manage, build and create relationships with local agencies and service providers.
+ Assist youth in navigating systems including providing court advocacy and transportation to medical appointments.
