Case Manager - Health Services Advocate

Support the vital mission of Parkdale Queen West Community Health Centre as a Case Manager focused on primary care and client advocacy. This role is centered in mid-west downtown Toronto.

This full-time, temporary position addresses community health needs for individuals facing barriers. Working alongside an interdisciplinary team, you will provide case management while advocating for essential client services. Your work will involve community collaboration to enhance resource availability and policy effectiveness for vulnerable groups.
