Case Manager

LaSante Health Center is seeking a compassionate and organized Case Manager to help patients address social, financial, and healthcare-related needs. The Case Manager will connect patients with community resources, public benefits, and support services while coordinating with the clinical care team.

Responsibilities:
+ Assess patients’ needs and develop individualized service plans.
+ Connect patients with food, housing, transportation, benefits, homecare, legal, and other community resources.
+ Assist patients with applications, referrals, and required documentation.
+ Follow up with patients and community organizations to ensure services are accessed.
+ Coordinate care with therapists, medical providers, and other team members.
+ Document all patient interactions, referrals, outreach, and follow-up activities accurately and promptly.
+ Track patient needs, referrals, deadlines, and progress.
