What will your responsibilities be?
In this role, you will work within the Discharge Assessment and Review Team (DART) at Doncaster Royal Infirmary, supporting discharges from hospital and the emergency department for adults aged 18 and above. You will carry out strength-based assessments under the Care Act 2014 and Mental Capacity Act 2005, focusing on safe discharge planning with a Home First ethos.
