Duties & Responsibilities:
Assess resident need, risk factors and barriers to stable ESF / community placement at referral. Identify and record baseline presentation of ‘challenging behaviors’ at referral and track progress during ESF placement.Identify existing services and supports (informal and formal) involved with resident.Seek to identifymostappropriate role and input to compliment and extend existing service provision in the interest of facilitating a successfulESF placementWork in conjunction with existing mental health providers when present – seek to clarify and agree complimentary role to meet residents’ needs and facilitate successful ESF placementParticipate in the creation of effective resident specific Person Centered Service Plans(PCSP) to be utilized by residents andPCSP Team members.Monitor use o...
