Catholic Health Home Care is looking for passionate, dedicated LPNs to provide comprehensive care to patients at home.

Team 2 Covers; Atlantic Beach, Baldwin, Cedarhurst, East Rockaway, Bellmore, Freeport, Hewlit, Inwood, Island Park, Lawrence, Lido Beach, Long Beach, Lynbrook, Merrick, North Bellmore, North Merrick, Oceanside, Point Lookout, Rockville Center , Valley Stream, Wantagh, Woodmere. 

  • 5x 8 hour shifts (8:30AM to 4:30PM) - Rotating Weekends
  • Great Healthcare benefits, and education reimbursement of $5,250 annually
  • Additional compensation for visits over your caseload

    • Job Details

    Our Home Care LPN’s:

  • Case manage, assess, plan, develop and implement a comprehensive home health care plan for their patients.
  • Measure vital signs, Administer IVs, change dressings, clean wounds as needed
