Intensive Case Manager – Wesley Hamilton Housing

This is a permanent, unionized, full‑time role. Pay rate $24.38 per hour, 40 hours per week.

Key Responsibilities

• Assist homeless individuals to locate, find, and attain permanent leased units.
• Document and input information into client files, both manually and in computer databases, ensuring accurate service statistics and client records.
• Deliver services to program participants who are single homeless adults.
• Develop, implement, and monitor case plans and sustainability plans.
• Maintain regular, effective communication with social housing providers, administrators, and private landlords to provide comprehensive support for housing stability.
• Facilitate case conference meetings; coordinate appropriate supports to establish a wrap‑around approach.
