Position Overview:

The Registered Nurse Case Manager is responsible for delivering high-quality, patient-centered hospice care while managing a caseload of patients across designated counties. This role focuses on direct patient care, interdisciplinary collaboration, care coordination, and documentation to ensure compassionate end-of-life support for patients and families.

Schedule & Work Details:

• Weekly Hour Guarantee: 36 hours


• Schedule: Monday through Friday, 8:00 AM to 4:30 PM


• On Call Requirements: On call 2–3 times per week and every 3rd weekend (Saturday and Sunday)


• Coverage Area: Mahoning, Columbiana, Trumbull, and portions of Portage counties including Garrettsville, Windham, Deerfield, Atwater, Nelson, Palmyra, and Paris


• Mileage Reimbursement: Yes, reimbursed between patient visits from first to last patient
