About the job Home Health Physical Therapist
Key Responsibilities:
- Conduct initial evaluations and assessments of patients' physical abilities and needs
- Develop individualized treatment plans based on patients' conditions and goals
- Provide hands‑on physical therapy treatments to patients in their homes
- Instruct patients and their families on exercises and activities to improve mobility, strength, and flexibility
- Monitor and document patients' progress and adjust treatment plans as needed
- Educate patients on injury prevention and home exercise programs
