Rutgers, The State University of New Jersey, is seeking a Habilitation Counselor for the Edison Outpatient Treatment within Rutgers University Behavioral Health Care. 

The purpose of the Habilitation Counselor position is to provide site and community-based case management and supported employment services to individuals receiving treatment services through UBHC’s Outpatient Dementia Clinic.


Among the key duties of this position are the following:

Supports individuals with Dementia and their family/caregiver to gain access to needed medical, social, education, mental health, addiction, co-occurring, and other services.

Demonstrates the ability to engage and establish a working relationship with and provide services to individuals who are diagnosed with a serious and persistent mental illness and co-occurring disorders who present engagement challenges.
